YOU MUST HAVE YOUR ATHLETE’S SCHOOL ID NUMBER AVAILABLE.

You must fill out each form.

• To access the online forms hold your cursor over the “Electronic Participation Forms” tab. You will get a drop down list of the forms.

• Click on the form name and fill out the information requested.

• To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box.

• Once you have filled out all of the information on each page you will have the opportunity to print the document.

• You will receive a confirmation email once the document has been reviewed by the PISD Athletic Training Staff.

• Students are required to use the Pre-participation – Physical Examination Form provided. NO OTHER PHYSICAL EXAM FORM WILL BE ACCEPTED. A new physical is required for all 7th-12th grade students, prior to the start of the new school year. Physicals dated prior to May 1st will not be accepted.

If your son/daughter has a history of Asthma, yet they do not carry an inhaler, please fill out the Inhaler Notification Form, and return it to the athletic trainer. You may print this form by selecting the Printable Forms Tab at the top of the page.

You are encouraged to fill out the history portion of the Medical History form prior to printing. Forms are also available from your campus and the Athletic Office.
